Educational Policies Committee Checklist

Checklist for Submissions

[ ] Complete all sections of the EPC Form pertinent to your proposal.

[ ] Obtain required signatures.

[ ] Obtain signatures from any other program directors or department chairs that would be affected by the changes.

[ ] Provide summary of reasons for the proposal and any fiscal effects.

[ ] Copy of a syllabus, using the syllabus format attached to the Proposal Form (syllabus format is also in faculty handbook).

[ ] A syllabus for each new course and any other course being affected by name change, description change, prerequisite change, etc.

[ ] Copy of minutes of the department faculty meeting in which the change was announced, discussed, and voted on. (Minutes must be document meeting date, members present, and recorder)

[ ] Send original proposal form, with signatures in blue ink and 15 hard copies to Paula McCoy, EPC Chair, Psychology Department, Wallace 923. (Print the 15 copies front and back to conserve paper).

[ ] Send one copy of Proposal in Microsoft Word format to mccoy.pk@suddenlink.net

Please note that the committee will adhere strictly to the submission deadline dates. It is very difficult to process, review, and attend to the paperwork in a reasonable and thoughtful fashion when items arrive at the last minute. Submissions recieved after the deadline date will be placed on the agenda for the following month.

FEBRUARY 15 CATALOG DEADLINE

Beginning 2004 - 2005 academic year, the WVSU and WVSCTC Catalogs will be published on an annual basis. Any departments wishing for their proposals to be approved, published, and fully implemented for the following fall term will need to submit proper forms and documentation before the February meeting, since the deadline is February 15 each academic year. Each department will need to submit one proposal in Word format. APC/EPC will submit all approved proposal forms electronically and must use consistent format.
